Job Title: Practice Nurse

We are seeking a highly skilled and experienced Practice Nurse to join our team at The Clinic Oakleigh Road. As a Practice Nurse, you will play a vital role in providing high-quality nursing services to our patients, working closely with our multidisciplinary team to deliver excellent patient care.

Key Responsibilities:
  • Provide nursing services to our practice population, including clinical duties, health promotion, and patient education.
  • Assess the needs of patients and develop individual treatment plans to promote health and wellbeing.
  • Implement and evaluate health promotion and wellbeing programmes, including chronic disease management and health assessments.
  • Work closely with the practice team to ensure compliance with practice policies and protocols, including infection prevention and control.
  • Support the team in dealing with clinical emergencies and participate in shared learning to improve patient care.
Requirements:
  • Registered Nurse with a current NMC PIN.
  • Previous experience in working in Primary Care.
  • Desirable: Experience in chronic disease management, Long Term Conditions, and Certification in immunisation and vaccination.
