We are seeking a self-motivated individual with health or social care experience to support people with dementia to live as independently as possible.

As a Dementia Link Worker, you will be part of our team in partnership with the NHS, providing excellent support to people living with dementia and their families/carers.

Key responsibilities include:

  • Providing specialist information and holistic advice to individuals and families affected by dementia.
  • Supporting people with dementia to access community resources and services.
  • Working collaboratively with healthcare professionals and other agencies to ensure seamless care.

Applicants must have previous experience working with vulnerable adults or older people.

In line with our safer recruitment practices, this post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

Salary: \u00a327,030.00 (moving to \u00a329,120.00 after 1 year)

Holiday Allocation: 25 days per year
