About the Role

A rewarding opportunity for passionate dementia professionals to join the HammondCare team as a Dementia Support Coach. Based out of our offices in VIC, this fixed-term full-time contract offers the chance to make a meaningful impact in the lives of individuals living with dementia and their families.

The role involves facilitating on-the-ground support, implementing relationship-based support strategies, and coaching clients, carers, and care staff to develop best practice support and care.

Key Responsibilities:

  • Collaborate with peers to develop the skills and expertise of aged care staff and carers to understand the person living with dementia and deliver exceptional support and care.
  • Support the implementation of recommendation plans on the ground, utilizing modelling and coaching techniques, and seek guidance from team leaders and consultants as required.
  • Facilitate and support the trial and implementation of brokerage items with clients and their carers, and seek guidance from team leaders and consultants as needed.

What We Need from You:

  • Strong organisational abilities, including effective time management and the ability to prioritise in a busy work environment and competing demands.
  • The ability to work both independently and as part of a team.
  • Demonstrated commitment to excellence in customer service principles, including conflict resolution, to ensure the consistent provision of exceptional service to internal and external customers.
  • A current driver's licence and safe driving record.
  • Willingsness to travel and work intrastate and interstate for up to five days at a time to support the delivery of dementia services.
  • Willingsness to work flexible hours at times and as required to meet the assessed needs of clients and carers.
  • Qualification in a relevant field, such as allied health, diversional therapy, enrolled nursing, or aged care, is highly desirable.
