About the Role

A rewarding opportunity for passionate professionals to work with HammondCare, a leading provider of aged care services. As a Dementia Support Coach, you will facilitate on-the-ground support, implement relationship-based support strategies, and model coaching techniques with clients, carers, and care staff.

Key Responsibilities

  • Work collaboratively with peers to develop the skills and expertise of aged care staff and carers to understand the person living with dementia and develop best practice support and care.
  • Support the implementation of recommendation plans on the ground, drawing on modelling and coaching techniques, and seek guidance from Team Leader, Dementia Consultant, and Senior Consultant as required.
  • Facilitate and support the trial and implementation of brokerage items with clients and their carers, and seek guidance from Team Leader, Dementia Consultant, and Senior Consultant as required.

Requirements

  • Strong organisational abilities, including effective time management and ability to prioritise in a busy work environment and competing demands.
  • The ability to work both independently and as part of a team.
  • Demonstrated commitment to excellence in customer service principles, including conflict resolution, to ensure the consistent provision of exceptional service to internal and external customers.
  • A current driver's licence and safe driving record.
  • Willingness to travel and work intrastate and interstate for up to five days at a time to support the delivery of DC services.
  • Willingness to work flexible hours at times and as required to meet the assessed needs of clients and carers.
  • Qualification in a relevant field, such as Allied Health assistant, Diversional Therapy, Enrolled Nursing (EN), Aged Care, Cert III or IV in Individual Support / Leisure and Health, or similar.
