About the Role

We are seeking a skilled Clinical Care Leader to join our team in Oldham. As an Advanced Clinical Practitioner, you will play a vital role in providing high-quality patient care and leading our team towards excellence.

Job Description

The successful candidate will be responsible for assessing and clinically examining patients with both minor and complex needs. You will have access to pathways and a broad Directory of Services (DoS) along with:

  • Direct access to a wide range of community-based services.
  • Referral rights to SDEC and secondary care.
  • Access to technology to support telephone and video consultations (full induction and training provided).
  • Facilities to see patients face-to-face (with support if you are expanding your scope of practice).

You will possess leadership qualities and provide support to the wider team, act as a positive role model and proactively identify areas for quality improvement.

Key Responsibilities
  • Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate healthcare service.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Refer and discharge safely ensuring that safety-netting advice is offered.
  • Triaging telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.
Non-Medical Prescribing

If you are employed in a prescribing capacity, you must only prescribe within your scope of practice and comply with the Gtdhealthcare NMP governance registration processes.

Delivering a Quality Service
  • Recognise and work within own competence and professional code of conduct as regulated by the NMC/HPC etc.
  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
  • Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
  • Deliver care according to NSF, NICE guidelines and evidence-based care.
  • Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.
Leadership, Personal and People Development
  • Taking responsibility for own learning and performance, participating in clinical supervision and acting as a positive role model.
  • Taking a lead role in planning and implementing changes within the area of care and responsibility.
  • Working with senior clinical and management teams to ensure sufficient staff of appropriate ability, quality and skill-mix is available to meet current and future service delivery.
Salary and Benefits

The salary for this role is between £54,000 to £65,250 per year, plus enhancements attached to KSF and dependent upon experience. We offer a comprehensive benefits package, including flexible working hours, family-friendly policies, and opportunities for career development.
