About the Role

The Betsi Cadwaladr University Health Board is seeking a skilled and motivated Mental Health Nurse to join our established integrated team. As a key member of our team, you will be responsible for the care co-ordination and case management of a defined caseload of clients with complex and challenging needs.

Main Duties

  • To be responsible for the care co-ordination of a designated caseload of clients with severe and enduring mental health problems.
  • To ensure that the Care and Treatment Plans are adhered to and that standards are met in line with the Mental Health Measure (Wales).
  • To act as a point of contact for GP's and other interface services to offer advice, support, and signposting to appropriate agencies.
  • To formulate and develop standard and complex outcome-focused care and treatment plans.
  • To ensure that a comprehensive risk assessment is completed and documented for actual and potential risk.

To be successful in this role, you will need to meet the essential criteria outlined in the attached job description and person specification. You will also have the ability to speak Welsh, although this is not essential.
