About the Role

The Community Mental Health Team at Betsi Cadwaladr University Health Board is seeking a skilled Mental Health Nurse to work within an established integrated, multi-disciplinary team. The successful post holder will undertake responsibility for the care co-ordination and case management of a defined caseload of clients with complex and challenging needs.

Main Responsibilities

  • To be responsible for the care co-ordination of a designated caseload of clients with severe and enduring mental health problems.
  • To ensure that the Care and Treatment Plans are adhered to and that standards are met in line with the Mental Health Measure (Wales) 2010.
  • To act as a point of contact for GP's and other interface services to offer advice, support and signposting to appropriate agencies.
