About the Role

We are seeking a skilled and motivated Mental Health Nurse to join our Community Mental Health Team. As a key member of the team, you will be responsible for providing high-quality care to individuals with severe and enduring mental health problems.

Main Responsibilities
  • Provide care coordination and case management for a defined caseload of clients with complex and challenging needs.
  • Develop and implement comprehensive care and treatment plans that meet the needs of clients.
  • Act as a point of contact for GPs and other interface services, providing advice, support, and signposting to appropriate agencies.
  • Assess and manage clinical risk, including completing comprehensive risk assessments and documenting findings.
  • Support clinical duties, such as assisting with the running of the depot/clozapine clinic and administering and supervising long-acting injections.
  • Complete physical health assessments and monitor side effects for patients prescribed Clozapine.
  • Assess and evaluate individuals in crisis, completing comprehensive assessments of their health and social care needs and agreeing the most appropriate course of action.
Requirements
  • Meet all essential criteria as per the attached job description/person specification.
  • Desirable criteria include experience working with individuals with severe and enduring mental health problems and proficiency in Welsh.
