Mental Health Nurse

The Community Mental Health Team at Betsi Cadwaladr University Health Board is seeking a skilled and motivated Mental Health Nurse to work within an established integrated, multi-disciplinary team. The successful post holder will undertake responsibility for the care co-ordination and case management of a defined caseload of clients with complex and challenging needs.
Main Duties- To be responsible for the care co-ordination of a designated caseload of clients with severe and enduring mental health problems.
- To ensure that the Care and Treatment Plans are adhered to and that standards are met in line with the Mental Health Measure (Wales).
- To act as a point of contact for GP's and other interface services to offer advice, support and signposting to appropriate agencies.
- To formulate and develop standard and complex outcome-focused care and treatment plans.
- To ensure that a comprehensive risk assessment is completed and documented for actual and potential risk.
- To be responsible for supporting clinical duties such as assisting with the running of the depot/clozapine clinic and the administration and supervision of long-acting injections.
- To be responsible for assessing individuals in Crisis, completing comprehensive assessments of an individual's health and social care needs and agreeing the most appropriate course of action.
