We are seeking an exceptional Primary Care Practitioner to join our Acute Care Team, working closely with Allied Healthcare Professionals to provide comprehensive care to patients. This role presents an excellent opportunity for a motivated and skilled individual to make a meaningful difference in the lives of our patients.
About the Role:
This is a rewarding position that requires a Certified General Practitioner (GP) who has completed their Certificate of Completed Training from October 2022 onwards. As a key member of our team, you will be responsible for assessing, diagnosing, and treating patients presenting to the Acute Care Team within our Primary Care Network.
You will work autonomously and collaboratively with other healthcare professionals to ensure the highest standards of patient care. Your dedication, expertise, and passion for delivering holistic care will contribute significantly to our mission of being the provider of choice in delivering person-centered care.
Key Responsibilities:
- Triage patients presenting to the surgery by telephone, online, and in-person.
- Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other healthcare workers within the organisation.
- Assessing the healthcare needs of patients with undifferentiated and undiagnosed problems.
- In consultation with patients and in line with current Practice disease management protocols, developing care plans for health and promoting healthy lifestyle strategies and education.
- Refer patients to secondary care and/or relevant organisations and internally within the PCN as required.
- Compiling and issuing computer-generated acute and repeat prescriptions in line with local and national guidelines.
Requirements:
- A GP CCT (Certificate of Completed Training) from October 2022 onwards.
- Clear, polite telephone manner.
- Flexible and cooperative attitude.
- Experience with clinical risk management.
- Experience with audit and quality improvement programmes.
Salary: £65,000 - £85,000 per annum (dependent on experience).
