About the Role

P3 Charity is seeking a dedicated Care Coordinator to join our team. As a Care Coordinator, you will provide intensive support to individuals facing homelessness and complex trauma. Your comprehensive package of holistic and practical support will ensure that individuals develop independent living skills needed to maintain a tenancy long term.

Key Responsibilities

  • Provide trauma-informed support to individuals in need
  • Work closely with the team and partner agencies to create a wrap-around package of support
  • Develop and maintain strong communication skills to effectively engage with individuals
  • Think creatively to recognize and address individual needs
