About the Role

We are seeking an experienced mental health practitioner to join our Specialist Community Forensic Team (SCFT) at Essex Partnership University NHS Foundation Trust. The successful candidate will work with a multidisciplinary team to provide care coordination for a small caseload of service users, supporting their transition from secure settings to community placements.

Key Responsibilities
  • Provide and enhance services for individuals in secure settings, pre, during, and post-discharge, to support timely transitions to community settings.
  • Contribute to the development of robust risk assessments and care plans, and attend regular inpatient reviews to monitor progress and address potential delays to discharge.
  • Work collaboratively with the MDT to support clients across Essex, and contribute to the day-to-day operational management of the SCFT.
Person Specification

We are looking for a candidate with a professional qualification in social work or mental health, and experience of working in community and forensic settings. The ideal candidate will have excellent communication and interpersonal skills, and be able to work effectively in a multidisciplinary team.

Essential qualifications include a recognised social work qualification, or a professional qualification in mental health with 12 months post-qualification experience. Desirable qualifications include HCR20 and certificate in cognitive behavioural therapy or brief solution-focused therapy.
