About the Role

We are seeking an experienced mental health practitioner to join our Specialist Community Forensic Team (SCFT) at Essex Partnership University NHS Foundation Trust. The successful candidate will work with a multidisciplinary team to provide care coordination for a small caseload of service users, supporting their transition from secure settings to community placements.

Key Responsibilities
  • Provide and enhance services for individuals in secure settings, pre, during, and post-discharge, to support timely transitions to community settings.
  • Contribute to the development of robust risk assessments and care plans, and attend regular inpatient reviews to monitor progress and address potential delays to discharge.
  • Work collaboratively with the MDT to support clients across Essex, and contribute to the day-to-day operational management of the SCFT.
Person Specification

We are looking for a candidate with a professional qualification in social work or mental health, and experience of working in secure environments. The ideal candidate will have a strong understanding of current legislation and government policy, and be able to demonstrate an up-to-date knowledge of community mental health services and forensic mental health services.

The successful candidate will be able to work on their own initiative, manage time effectively, and maintain professional relationships within the MDT. They will also have excellent communication and interpersonal skills, and be able to inspire confidence in service users, carers, and junior staff.
