About the Role

We are seeking an experienced mental health practitioner to join our Specialist Community Forensic Team (SCFT) at Essex Partnership University NHS Foundation Trust. The successful candidate will work with a multidisciplinary team to provide care coordination for a small caseload of service users, supporting their transition from secure settings to community placements.

Key Responsibilities
  • Provide and enhance services for individuals in secure settings, pre, during, and post-discharge, to support timely transitions to community settings.
  • Contribute to the development of robust risk assessments and care plans, and attend regular inpatient reviews to monitor progress and address potential delays to discharge.
  • Work collaboratively with the MDT to support clients across Essex, and contribute to the day-to-day operational management of the SCFT.
Person Specification

We are looking for a professional with a recognized Social Work qualification, or a Professional Qualification in Mental Health (RMN) with 12 months post-qualification experience. The ideal candidate will have evidence of post-qualification training, and a strong understanding of current legislation and government policy.

Desirable qualifications include HCR20, Certificate in Cognitive Behavioural Therapy, or Brief Solution Focused Therapy. The successful candidate will also have excellent communication and interpersonal skills, and be able to work effectively in a fast-paced environment.
