We are seeking an experienced and skilled Advanced Clinical Practitioner to join our team at Havering Health. As a key member of our primary care network, you will play a vital role in delivering high-quality, patient-focused care to our communities.
About the RoleThe successful candidate will be a registered nurse with significant experience in primary care and community settings. You will have a strong background in clinical leadership, with a proven track record of managing minor illnesses, minor ailments, and minor injuries. Your expertise will enable you to provide expert care, including assessment and diagnostic skills, for patients within the general practice.
You will be responsible for proactively identifying, diagnosing, and managing treatment plans for patients at risk of developing long-term conditions. You will also support patients in adopting healthy lifestyles and applying principles of self-care. Additionally, you will assess, identify, and refer patients presenting with mental health needs in accordance with the NSF for Mental Health.
As a member of our team, you will work collaboratively with carers and other professionals to deliver high-quality care. You will also be responsible for planning and delivering training to other professionals and developing protocols and clinical guidelines.
Person Specification- Significant relevant experience at post-registration level
- Recent primary care/community experience
- ACP-led management of minor illness, minor ailments, and minor injuries
- Good management/project management skills
- Experience showing clinical leadership skills
- ACP-led triage
- Interpretation of blood results and acting appropriately with abnormal findings
- Working collaboratively with carers and other professionals
- Experience of working in a multi-agency and/or multi-disciplinary team
- Planning and delivery of training to other professionals
- Developing protocols and clinical guidelines
- Recognised Masters qualification in Advanced Clinical Practice relevant to the clinical caseload (e.g. frailty, older adults, primary care)
- Registration with an appropriate healthcare governing body and current licence to practice (e.g. NMC, HCPC etc)
- Mentor/teaching qualifications
- Extended/independent prescribing qualification (where relevant to background profession)
