About the Role

We are seeking an experienced Advanced Clinical Practitioner to join our team at Havering Health. As a key member of our primary care network, you will play a vital role in delivering high-quality, patient-focused services to our community.

The successful candidate will be a registered nurse with significant experience in primary care, possessing strong clinical leadership skills and the ability to manage minor illnesses, minor ailments, and minor injuries. You will be responsible for leading the delivery of practice nursing services, ensuring the needs of patients are prioritized, and working collaboratively with carers and other professionals.

Key responsibilities will include:

  • Managing and assessing risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients.
  • Monitoring work areas and practices to ensure they are safe and free from hazards and conform to health, safety, and security legislation, policies, procedures, and guidelines.
  • Using technology and appropriate software as an aid to management in planning, implementation, and monitoring of care, presenting and communicating information.
  • Reviewing and processing data using accurate Read codes in order to ensure easy and accurate information retrieval for monitoring and audit processes.
  • Undertaking mentorship for more junior staff, assessing competence against set standards.
  • Disseminating learning and information gained to other team members in order to share good practice and inform others about current and future developments.

Person Specification

We are seeking a candidate with:

  • Significant relevant experience at post-registration level.
  • Recent primary care/community experience.
  • ACP-led management of minor illness, minor ailments, and minor injuries.
  • Good management/project management skills.
  • Experience showing clinical leadership skills.
  • ACP-led triage.
  • Interpretation of blood results and acting appropriately with abnormal findings.
  • Working collaboratively with carers and other professionals.
  • Experience of working in a multi-agency and/or multi-disciplinary team.
  • Planning and delivery of training to other professionals.
  • Developing protocols and clinical guidelines.
Qualifications

We are seeking a candidate with:

  • Recognized Masters qualification in Advanced Clinical Practice relevant to the clinical caseload (e.g., frailty, older adults, primary care).
  • Registration with an appropriate healthcare governing body and current license to practice (e.g., NMC, HCPC, etc.).
  • Mentor/teaching qualifications.
  • Extended/independent prescribing qualification (where relevant to background profession).
