About the Role

We are seeking an experienced Advanced Clinical Practitioner to join our team at Havering Health. As a key member of our primary care network, you will play a vital role in delivering high-quality, patient-focused services to our community.

The successful candidate will have significant relevant experience at post-registration level, with recent primary care/community experience and a proven track record of clinical leadership. You will be responsible for managing minor illness, minor ailments, and minor injuries, as well as interpreting blood results and acting appropriately with abnormal findings.

We are looking for a candidate who is able to work collaboratively with carers and other professionals, with experience of working in a multi-agency and/or multi-disciplinary team. You will also have the ability to plan and deliver training to other professionals and develop protocols and clinical guidelines.

Key Responsibilities
  • Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients.
  • Monitor work areas and practices to ensure they are safe and free from hazards and conform to health, safety, and security legislation, policies, procedures, and guidelines.
  • Use technology and appropriate software as an aid to management in planning, implementation, and monitoring of care, presenting, and communicating information.
  • Review and process data using accurate Read codes in order to ensure easy and accurate information retrieval for monitoring and audit processes.
  • Undertake mentorship for more junior staff, assessing competence against set standards.
  • Disseminate learning and information gained to other team members in order to share good practice and inform others about current and future developments.
Person Specification
  • Recognised Masters qualification in Advanced Clinical Practice relevant to the clinical caseload (e.g., frailty, older adults, primary care).
  • Registration with an appropriate healthcare governing body and current licence to practice (e.g., NMC, HCPC, etc.).
  • Mentor/teaching qualifications.
  • Extended/independent prescribing qualification (where relevant to background profession).
