As a Care Facilitator, you will be part of our dynamic team, working closely with clinicians, administrators, and patients to ensure seamless care coordination and communication.

Your primary responsibilities will include acting as a link between clinicians and clinical correspondence, managing patient communication, and facilitating the provision of preventative healthcare. You will also provide secretarial support and cover as needed. To succeed in this role, you will require strong communication and interpersonal skills, experience working with members of the public, and excellent IT skills.
