We are seeking a highly skilled and dedicated Children and Young People's Practitioner to join our team at Salutem Shared Services. As a Positive Behavioural Support Specialist, you will play a vital role in delivering high-quality support to individuals with complex needs.
Key Responsibilities- Implement and deliver Positive Behavioural Support and person-centred planning to maximise service users' quality of life.
- Act as a liaison on behavioural matters between care and the behavioural forum.
- Directly provide support to help develop and maintain high-quality Person-Centred Plans (PCP), 'My Plans' and Health Files.
- Take on the responsibility of the shift leader when there is no Registered Manager or Deputy Manager present.
- Attend internal and/or external case conferences, reviews and meetings as required.
