About the Role

We are seeking a skilled and dedicated Community Mental Health Nurse to join our Older Adult Community Mental Health Team. The successful candidate will work with patients who have complex functional and organic mental health needs, providing evidence-based interventions and support.

The core working hours for this role are Monday to Friday (9am - 5pm), with the opportunity to work flexibly. You will be part of an integrated team consisting of experienced professionals, including Community Mental Health Nurses, Occupational Therapists, Consultant Psychiatrists, Junior Medics, and Support Workers.

You will be supported by the Team Manager, who offers regular supervision and appraisal sessions to help you develop your skills and knowledge. Our team prioritizes patient-centered care and offers ongoing continuing professional development opportunities.

Main Responsibilities

  • Manage a caseload of patients with complex mental health needs, using solution-focused principles to assess, plan, implement, and evaluate interventions.
  • Work collaboratively with patients, carers, and other local agencies to achieve agreed outcomes and goals.
  • Maintain accurate and timely clinical records, ensuring compliance with NHS standards.
  • Provide leadership and guidance to junior staff members through supervision and appraisal.
