About the Role:

We are seeking a dedicated and compassionate Support Worker to join our team at Community Integrated Care. As a Support Worker, you will play a vital role in supporting an individual with a brain injury to live independently in their own home. Your primary responsibility will be to provide personal care and support to enable the individual to maintain their independence and quality of life.

Key Responsibilities:

  • Provide personal care and support to the individual, including assistance with daily living tasks, mobility, and communication.
  • Develop and maintain a strong relationship with the individual, based on trust, respect, and empathy.
  • Work collaboratively with the individual's care team to ensure their needs are met and their goals are achieved.
  • Contribute to the development and implementation of the individual's care plan, in conjunction with the care team.
  • Provide emotional support and companionship to the individual, promoting their physical and mental well-being.

Requirements:

  • A caring and compassionate nature, with a strong commitment to supporting individuals with disabilities.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals and their families.
  • Ability to work independently and as part of a team, with a flexible approach to working hours and tasks.
  • Basic knowledge of personal care and support techniques, with a willingness to learn and develop your skills.
  • Reliability, honesty, and a strong work ethic, with a commitment to maintaining confidentiality and upholding the values of Community Integrated Care.
