Care Coordinator

At Community Integrated Care, we are seeking a dedicated and caring individual to support our client, J. In this role, you will work one-to-one with J, assisting her with personal care and daily activities, while promoting her independence and well-being.
J is a client with a brain injury and requires support with mobility. You will be responsible for providing transportation to appointments and outings, using her adapted car. This is a great opportunity to make a real difference in J's life and support her in achieving her goals.
