Job Summary

We are seeking a skilled Care Home Liaison Nurse to provide a comprehensive community mental health liaison service for individuals with dementia and other mental health problems over 65 years residing in Care Homes across South Ayrshire.

The successful candidate will work as part of the Community Mental Health Team, providing specialist psychiatric nursing assessment, expert advice, information, and guidance to care home staff, patients, and carers.

Key Responsibilities
  • Provide specialist psychiatric nursing assessment and expert advice to care home staff, patients, and carers.
  • Work collaboratively with the Community Mental Health Team to deliver a comprehensive community mental health liaison service.
  • Develop and maintain relationships with care home staff, patients, and carers to ensure effective communication and support.
  • Contribute to the development and implementation of care plans that meet the needs of individuals with dementia and other mental health problems.
  • Participate in multidisciplinary team meetings to discuss patient care and develop strategies for improving mental health outcomes.
Requirements
  • Registered Nurse on Part 3 of the NMC Register.
  • Degree or relevant postgraduate or appropriate experience relevant to the job purpose.
  • Evidence of Continuous Professional Development.
  • Excellent clinical knowledge.
Working Arrangements

This role requires a UK driving licence and car ownership. The working week will be 15 hours, with the option to work 37.5 hours (pro rata for part-time staff) until the department can meet the requirements without impacting patient safety.

NHS Ayrshire and Arran is committed to implementing the 37-hour working week across all areas as soon as it is safe to do so.
