About the Role

NHS Ayrshire and Arran is seeking a skilled Care Home Liaison Nurse to join our team. As a key member of our Community Mental Health Team, you will provide a comprehensive community mental health liaison service for people with dementia and other mental health problems over 65 years who reside in Care Homes across South Ayrshire.

Key Responsibilities
  • Provide specialist psychiatric nursing assessment and expert nursing advice, information, and guidance to care home staff, patients, and carers.
  • Work collaboratively with the Community Mental Health Team to deliver high-quality care and support to individuals in Care Homes.
  • Develop and maintain strong relationships with care home staff, patients, and carers to ensure effective communication and coordination of care.
  • Contribute to the development and implementation of care plans that meet the unique needs of individuals in Care Homes.
Requirements
  • Registered Nurse on Part 3 of the NMC Register.
  • Degree or relevant postgraduate or appropriate experience relevant to the job purpose.
  • Evidence of Continuous Professional Development.
  • Excellent clinical knowledge.
  • UK Driving Licence/Car Ownership: UK Car licence is essential to this role.
Working Arrangements

This is a part-time role, working 15 hours per week. You will be required to work collaboratively in a safe, caring, and respectful way, in line with our organisational values.

From 1 April 2024, the working week for NHS Agenda for Change workers in Scotland will be reduced. Full-time hours will reduce from 37.5 to 37 hours (pro rata for part-time staff) without loss of earnings. NHS Ayrshire and Arran will implement this change, and you may be required to work 37.5 hours per week (pro rata for part-time staff) until the department can meet the requirements without impacting patient safety.
