About the Role

We are seeking a skilled and compassionate Community Nurse to join our dedicated team at Lincolnshire Community Health Services NHS Trust. As a Community Nurse, you will play a vital role in delivering high-quality care to patients in their own homes and care homes.

Key Responsibilities
  • Assess patient needs and develop personalized care plans
  • Deliver care and support to patients in their own homes and care homes
  • Work collaboratively with multidisciplinary teams to ensure effective care coordination
  • Provide education and support to patients and their families
  • Contribute to the development and implementation of care pathways and protocols
Requirements
  • Current 1st level UK NMC adult nurse registration
  • Evidence of continuous professional development
  • Evidence of V100 or V150 Community Practitioner prescribing course undertaken
  • Experience of assessment, care planning, and care delivery
  • Experience of delegation and supervision of others
  • Evidence of the application of research and evidence-based care
