About the Role

We are seeking a skilled and compassionate Apprentice District Nurse to join our dedicated team at Lincolnshire Community Health Services NHS Trust. As a community nurse, you will play a vital role in delivering high-quality care to patients in their own homes and care homes.

Main Responsibilities
  • Assess patient care needs and develop personalized care plans
  • Deliver evidence-based care using innovative practices
  • Work collaboratively with multidisciplinary teams to ensure effective care coordination
  • Provide wound management, continence care, palliative care, and medication administration

Person Specification

We are looking for a candidate with:

  • Current 1st level UK NMC adult nurse registration
  • Evidence of continuous professional development
  • Evidence of V100 or V150 Community Practitioner prescribing course undertaken
