About the Role

We are seeking an experienced and compassionate Adult Care Team Leader to join our team at Hazel House, a specialist care provider for individuals with autism, complex needs, and learning disabilities.

As a key member of our team, you will be responsible for the wellbeing of our occupants, ensuring their physical and emotional needs are met. This will involve chaperoning them to appointments and activities, completing relevant paperwork, and undertaking household tasks.

You will also lead, train, and develop new and existing support workers within your team, helping to maintain a skilled and knowledgeable team that delivers exceptional care.

Key Responsibilities
  • Lead and manage a team of support workers to ensure high-quality care is delivered
  • Develop and implement care plans that meet the unique needs of each occupant
  • Chaperone occupants to appointments and activities, ensuring their safety and well-being
  • Complete relevant paperwork and maintain accurate records
  • Undertake household tasks, including cleaning and laundry
  • Administer medication, as required
  • Lead training and development sessions for support workers
Requirements
  • NVQ Level 3 Health and Social Care (Adults) or equivalent qualification
  • Full UK driving licence and access to a vehicle
  • Previous experience in a similar setting, with a passion for delivering exceptional care
  • Effective communication skills, both verbal and written
  • Ability to work independently and as part of a team
