About the Role

We are seeking a highly skilled and compassionate Adult Care Team Leader to join our team at Hazel House, a specialist residential care facility for individuals with autism, complex needs, and learning disabilities.

Key Responsibilities
  • Lead and manage a team of support workers to ensure the highest standard of care and support for our residents.
  • Develop and implement individualized care plans to meet the unique needs of each resident.
  • Chaperone residents to appointments and activities, complete relevant paperwork, and undertake household tasks as required.
  • Administer medication and provide emotional support to residents as needed.
  • Collaborate with the deputy manager to maintain a skilled and knowledgeable team, ensuring the delivery of exceptional care and support.
Requirements
  • NVQ Level 3 Health and Social Care (Adults) or equivalent qualification.
  • Full UK driving license and access to a vehicle.
  • Previous experience in a similar setting with a passion for making a difference in the lives of vulnerable individuals.
  • Effective communication skills, both verbal and written.
  • Patience, resilience, and a calming presence.
