Job Summary
We are seeking a skilled Community Mental Health Social Worker to join our team at Neath Port Talbot County Borough Council. As a key member of our Community Mental Health Team, you will work closely with individuals, families, and communities to provide high-quality mental health services.
Key Responsibilities
- Conduct assessments, reviews, and care planning to ensure the best possible outcomes for service users.
- Work collaboratively with multidisciplinary teams, including healthcare professionals, to deliver comprehensive care.
- Develop and maintain strong relationships with service users, their families, and carers to ensure a person-centered approach.
- Stay up-to-date with the latest best practices in mental health and apply this knowledge to improve service delivery.
Requirements
- Professional registration as a social worker with Social Care Wales.
- Experience working in mental health, preferably in a community setting.
- Knowledge of the Mental Health Act 1983 (amended 2007) and the Mental Health Measure (Wales) 2010.
- Excellent communication and interpersonal skills.
