Community Care Specialist

We are looking for a skilled and dedicated Allied Healthcare Professional to provide expert interventions and rehabilitation as part of our dynamic, tight-knit team that reacts quickly when people are in crisis. The successful candidate will be responsible for planning programmes of care to promote health gain and maximise independence, actively case managing patients through their journeys.
Key Responsibilities:- Provide expert interventions and rehabilitation to patients in their own homes or in a community setting.
- Develop and implement individualised care plans to promote health gain and maximise independence.
- Collaborate with other healthcare professionals to ensure seamless care delivery and coordination.
- Registered Allied Healthcare Professional with HCPC registration.
- Experience of working in a community or in-patient setting for a minimum of 2 years post-registration.
- Demonstrated experience of recent Continuous Professional Development.
