Job Overview:
We are seeking an exceptional Registered Nurse to join our team as a Practice Nurse. This is a fantastic opportunity to deliver high-quality patient care and make a real difference in the lives of our patients.
About the Role:
This full-time position involves delivering patient care services, including chronic disease management, immunisations, and screening programs. You will work collaboratively with our healthcare team to ensure seamless delivery of services.
Key Responsibilities:
- Administer vaccinations and immunisations according to national and local guidelines
- Manage chronic diseases, including asthma, hypertension, and COPD, and initiate diagnosis processes
- Maintain accurate disease registers and health promotion programs
- Conduct travel health clinics and provide cervical and sexual health screening
- Collaborate with the wider practice team to achieve excellent patient care outcomes
Requirements:
- Registered Nurse with the Nursing and Midwifery Council
- Postgraduate diploma or degree in Chronic Disease Management
- Family planning qualification and competency in using Emis
- Excellent communication and time management skills
- A commitment to ongoing professional development and evidence-based practice
Salary and Benefits:
