NHS Practice Nurse

We are seeking a skilled Practice Nurse to join our dynamic team at the NHS. The successful candidate will be responsible for delivering high-quality patient care and managing chronic diseases in accordance with national guidelines.
This is an excellent opportunity for a qualified Nurse to work in a fast-paced primary care environment, utilizing their clinical knowledge and skills to provide outstanding care to our patients.
The estimated salary for this role is £40,000 - £45,000 per annum, depending on experience.
Main Responsibilities:
- Deliver high-quality patient care, including chronic disease management and immunizations.
- Work collaboratively with the wider Practice team and Primary Care Network.
- Manage chronic diseases, including asthma, hypertension, COPD, CVA-TIA, CHD, and CHD risk assessment and management.
To be successful in this role, you will need:
- A Registered Nurse qualification with the Nursing and Midwifery Council.
- Post-graduate diploma or degree in Chronic Disease Management.
- Clinical knowledge in areas such as ECGs, venepuncture, and immunizations.
