Mental Health Support Worker Lead

As a member of our Community Mental Health Team, the Care Coordinator - Vulnerable Adults will play a crucial role in supporting vulnerable adults through their recovery journey.
Main responsibilities include:
- Conducting comprehensive assessments and developing support plans;
- Preparing detailed reports and engaging in face-to-face interactions;
- Effective management of assigned cases to meet performance targets.
The objective of our team is to enhance the wellbeing of vulnerable adults by offering personalized care and support.
About Your Qualifications:
To be suitable for this position, you should hold:
- Minimum two years of experience as a social worker;
- A recognized degree in social work (Degree/DipSW/COSW);
- Ability to work in a demanding environment with commitment to delivering exceptional care;
- A valid UK driving license for traveling between locations.
Salary Information:
The estimated hourly compensation for this role is £18.00.
