We are looking for an Advanced Cardiac Nurse Practitioner to join our team in North Wales. As an experienced nurse with a background in cardiology, you will be responsible for providing high-quality patient care and leading on cardiac rehabilitation interventions.

About the Role

You will work closely with patients and their families to promote recovery and self-management, using your specialist knowledge and expertise to make a real difference in their lives.

  • You will lead on and implement cardiac rehabilitation interventions, working across various settings to ensure seamless patient care.
  • You will contribute to the delivery of cardiac rehabilitation care, working collaboratively with other healthcare professionals to achieve the best possible outcomes.

This is a permanent, part-time position with a salary of £40,500 - £57,500 per year, depending on the person specification matched.
