About the Role

We are seeking a skilled and compassionate Community Mental Health Practitioner to join our team at Essex Partnership University NHS Foundation Trust. As a key member of our Op COURAGE - The Integrated Veterans Mental Health and Wellbeing Service, you will play a vital role in providing high-quality care to members of the Armed Forces community.

Key Responsibilities
  • Manage a caseload of community-based patients, assessing their needs, developing care plans, and coordinating care to ensure the best possible outcomes.
  • Provide face-to-face and remote contact with patients, building strong therapeutic relationships and empowering them to take control of their mental health.
  • Stay up-to-date with current practices, local and national policies, and agendas in relation to Veterans mental health, ensuring that care plans and risk assessments are completed and maintained in line with set key performance indicators.
  • Liaise with relevant statutory and non-statutory services involved in the provision of care, advocating for the needs of the Armed Forces community and ensuring seamless transitions between services.
  • Develop and maintain knowledge of qualified service providers within the local region, ensuring that patients receive the best possible care.
Requirements
  • RMN/OT/SW registration and post-registration education related to mental health.
  • Experience working within a community setting, with the ability to assess for evidence of clinical mental health disorders and formulate treatment plans.
  • Ability to risk assess and provide clear risk management plans, ensuring the safety and well-being of patients.
