About the Role

We are seeking a passionate and experienced Mental Health Practitioner to join our team in providing high-quality care to the Armed Forces community. As a Community Mental Health Practitioner, you will be responsible for assessing the needs of our patients, developing and implementing care plans, and providing face-to-face and remote contact to support their mental health and wellbeing.

Key Responsibilities
  • Manage a caseload of community-based patients and provide individualized care plans to meet their unique needs.
  • Provide face-to-face and remote contact with patients to assess their mental health and wellbeing, and develop and implement care plans to support their recovery.
  • Work closely with other healthcare professionals and community organizations to ensure seamless care and support for our patients.
  • Advocate for the needs of the Armed Forces community and empower individuals to take control of their mental health and wellbeing.
  • Stay up-to-date with current practices, local and national policies, and agendas in relation to Veterans mental health.
Requirements
  • RMN/OT/SW registration and post-registration education related to mental health.
  • Experience working within a community setting and lone working.
  • Ability to assess for evidence of clinical mental health disorders or those at risk of, and formulate a treatment plan.
  • Ability to risk assess and provide a clear risk management plan.
