About the Role

As a Senior Community Mental Health Practitioner, you will be responsible for assessing the mental health needs of veterans and developing personalized care plans to support their recovery.

You will work closely with patients, their families, and other healthcare professionals to provide holistic care and address the unique challenges faced by veterans.

Our team is committed to delivering high-quality care, and we offer ongoing training and development opportunities to ensure our staff stay up-to-date with the latest practices and research.

Key Responsibilities
  • Assess and manage a caseload of community-based patients, developing and implementing care plans to meet their individual needs.
  • Provide face-to-face and remote contact with patients, building strong therapeutic relationships and empowering them to take control of their mental health.
  • Collaborate with other healthcare services and organizations to ensure seamless care and support for veterans.
  • Stay up-to-date with current practices, local and national policies, and agendas related to veterans' mental health.
Requirements
  • RMN/OT/SW registration and post-registration education related to mental health.
  • 2 years post-registration experience in mental health, with a focus on community-based care.
  • Personal or professional experience working with the Armed Forces community.
