About the Opportunity

We are seeking a Band 6 Nurse to lead one of our Community Nursing teams at Hawkhurst Community Hospital, Weald. This role involves working on a 1:4 ratio, with weekends off during the week.

As Team Leader, you will be part of two teams supporting patients registered with GP practices in the Hawkhurst area and surrounding villages within the Weald Primary Care Network.

Please note that this is a community-based role, requiring a driving licence and access to a vehicle.

Main Responsibilities
  • Support implementation of local and national agendas for health
  • Prioritise care delivery in a professional manner, considering Trust policies and procedures
