Care Coordinator

At St Andrew's, we are seeking a Part-time Senior Staff Nurse to join our Community Service team.
We offer the chance to make a real difference in the lives of individuals with mental health needs and learning disabilities. As a Care Coordinator, you will work across our STS (Supportive Transition Service) sites, providing specialist nursing care and support to our service users.
This role involves working a part-time schedule of 22 hours per week, with a shift pattern that includes mornings and afternoons, four days out of seven. You will be required to maintain accurate records, develop and implement care plans, and participate in multi-disciplinary teams to achieve the best possible outcomes for our service users.
The salary for this role is up to £39,496 per annum, pro rata, plus shift enhancements. This reflects the importance we place on attracting and retaining skilled professionals who can provide high-quality care and support.
