About the Role

We are seeking a highly motivated and empathetic Discharge Support Worker to join our team at Livewell Southwest. As a Discharge Support Worker, you will play a vital role in supporting individuals with mental health needs to achieve their recovery goals and prepare for discharge from hospital.

Key Responsibilities
  • Assist the multidisciplinary team in delivering physical, social, and psychological care to individuals with mental health illness.
  • Provide support to an allocated group of individuals with more complex needs, promoting their recovery and maintaining them in their community environment.
  • Assist the multidisciplinary team to assess, plan, implement, and evaluate care, and implement discharge plans.
  • Deliver physical, social, and psychological care to individuals with mental health difficulties, working in hospital and community settings.
  • Liaise with relatives, carers, and other agencies regarding discharge plans.
  • Provide information on health promotion and support individuals to maintain their homes.
  • Be an active member of the provision of care and maintain confidentiality at all times.
Requirements
  • Motivated and empathetic individual with a passion for supporting individuals with mental health needs.
  • Effective communication skills and ability to work as part of a team.
  • Ability to work in stressful conditions and access the community either by bus or the pool car.
  • Work a shift pattern that includes 10 hours days.
  • Knowledge of personal safety, confidentiality, de-escalation skills, and mental health legislation.
  • Ability to demonstrate knowledge of how to listen and communicate effectively, build community links, and spend time effectively.
