About the Role

We are seeking a Social Worker to join our core care team, supporting patients with End Stage Renal Disease through their treatment journey. As a vital member of our team, you will help patients understand their rights and responsibilities, guide them in managing physical, mental, emotional, and financial demands, and advocate for their needs.

Responsibilities

  • Build meaningful relationships with patients and their families in an intimate outpatient setting.
  • Collaborate with the care team to deliver patient-centered care and support.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions.

Requirements

  • Master's degree in Social Work (MSW) with a specialization in clinical practice.
  • Demonstrated knowledge of government and private insurance programs.
  • Basic computer skills in MS Word, Excel, PowerPoint, and Outlook, with functional proficiency in DaVita-specific applications within 60 days.

Compensation Range

The estimated salary for this role is $65,000 - $85,000 per annum, depending on qualifications, experience, and performance. We offer a comprehensive compensation package, which includes base pay, bonuses, and benefits.
