Care Coordination Specialist

Our vision at Sussex Community is to provide excellent care at the heart of the community. We are seeking a skilled Care Coordination Specialist to support our Site Capacity & Flow Lead in managing patient flow from acute providers to and across SCFT Intermediate Care Units.
About the RoleThe successful candidate will ensure all bed capacity across the Trust is utilised whilst ensuring patients experience safe, high quality care throughout their patient journey. This includes working collaboratively with external stakeholders and partner organisations to address urgent care local pressure, system surges and escalation.
The estimated salary for this role is £45,000 - £55,000 per annum, depending on experience.
