At Tameside Community Safety Partnership, we are seeking a highly skilled and innovative Care Management Specialist to support the growth of our Mental Health Team in Adult Social Care.

The successful candidate will contribute to the development of specialist care models for adults with mental illness, ensuring person-centred and legally compliant practices.

This role involves supporting leadership, developing specialist services, effective management, practice improvement & safeguarding, collaboration & communication, CQC compliance, and operational excellence.

To be considered for this position, you must have a Level 6 social work degree or equivalent qualification, strong background in Adult Social Care, experience working with adults with mental illness and an understanding of transitions.

A proven track record of leadership experience, particularly in managing service change and redesign, is essential. Excellent communication and interpersonal skills, with the ability to motivate and develop teams, are also required.

The estimated salary for this role is £44,711 - £48,710 per annum, with a professional grade development bar at SCP 37 (£46,731).
