Social Worker Position for Adults

This exciting opportunity is for an experienced Adult's Social Worker to join our Ambitious, Innovative, and Supportive Hospital in-reach Adult Social Care Pathway. You will be working with adults with complex mental health needs and their carers, advocating and developing support services tailored to individual needs.
Key Responsibilities- Conduct assessments, develop and implement care plans, and evaluate service user outcomes.
- Collaborate with a multi-disciplinary team, using evidence-based interventions to deliver effective support services.
- Manage your own workload, prioritize tasks, and maintain accurate records.
- A relevant social work qualification, current registration, and significant post-qualification experience in mental health settings.
