Adults Social Care Specialist

We are seeking an experienced Adult's Social Worker to join our innovative Hospital in-reach Adult Social Care Pathway. As a key member of our team, you will be advocating and working with adults with complex mental health needs and their carers, providing active involvement in assessment, development, implementation, and evaluation of service user care.
Main Duties- Provide support and services appropriate to individuals' needs, utilizing strength-based approaches.
- Work collaboratively and effectively using evidence-based interventions, as part of a multi-disciplinary team.
- Be responsible for managing your own workload, dealing with challenging behavior, and visiting people at their homes.
- A professional social work qualification, registration with the regulatory body, and experience in mental health settings.
- Able to hold or achieve Approved Mental Health Professional and Best Interest Assessor status.
- Good keyboard/IT skills, commitment to equal opportunities, and ongoing personal and professional development.
