Job Title: Complex Needs Case ManagerWe are seeking a dedicated Complex Needs Case Manager to join our team in Hammersmith and Fulham. As a key member of our service, you will play a crucial role in supporting individuals with complex needs and mental health crises aged 18+. Your contributions will directly influence the lives of our service users, guiding them towards a path of empowerment and independence.
Key Responsibilities:- Actively monitor the health, well-being, safety and security of the service and its users, ensuring all concerns are followed up promptly and in line with procedures.
- Ensure that administration is done in good time, that detailed, concise and accurate records are created contemporaneously, and that confidentiality is appropriately maintained.
The ideal candidate will possess good communication skills, both written and spoken, and demonstrate a flexible and proactive approach to work. Experience of working with people with mental health and complex needs is essential. You will also have knowledge of safeguarding policies and procedures.
Salary: £25,000 - £30,000 per annum-
