Job Summary

We are seeking a skilled and experienced Nurse Practitioner/Clinical Practitioner/Extended Care Practitioner to join our team at Clifton Road Surgery in Rugby. As a key member of our multidisciplinary team, you will be responsible for providing high-quality patient-centered care to our diverse patient population.

Main Duties and Responsibilities

As an Advanced Nurse Practitioner/Clinical Practitioner/Extended Care Practitioner, you will be expected to work autonomously and as part of a team to provide safe, evidence-based, and cost-effective patient care. Your key responsibilities will include:

  • Triage patients and provide clinical assessments, diagnoses, and treatment plans
  • Make referrals to secondary care and social services as needed
  • Provide clinical advice to reception staff and other healthcare professionals
  • Work collaboratively with the multidisciplinary team to promote integrated and seamless pathways of care
  • Contribute to the practice's quality targets and initiatives to improve patient services

Requirements and Qualifications

To be successful in this role, you will need to be a registered Nurse Practitioner/Clinical Practitioner/Extended Care Practitioner with a prescribing qualification and clinical skills. A degree in healthcare or equivalent experience is desirable. You must be able to work autonomously and as part of a team, and have excellent communication and interpersonal skills.
