About the Role

We are seeking a highly motivated and compassionate Community Mental Health Nurse to join our dedicated team at Gloucestershire Health and Care NHS Foundation Trust. As a key member of our multi-disciplinary team, you will play a vital role in providing high-quality care to older adults with severe mental health conditions.

Key Responsibilities
  • Coordinate care for a designated caseload of older adults with severe mental health conditions.
  • Work within the Care Programme Approach (CPA) and associated countywide risk procedures.
  • Travel independently to provide care and support in the community.
  • Utilize IT skills to maintain accurate records and communicate with colleagues.
  • Develop a working knowledge of legislation regarding safeguarding vulnerable adults, the Mental Health Act, and the Mental Capacity Act.

We are looking for a candidate with:

Experience in mental health care, particularly with older adults.Qualifications in nursing or a related field.Desirable: experience in community mental health and later life interventions.
