About the Role

We are seeking a highly motivated and caring Community Mental Health Nurse to join our multi-disciplinary team at Gloucestershire Health and Care NHS Foundation Trust. As a key member of our team, you will be responsible for providing high-quality care to older adults with severe mental health conditions.

Key Responsibilities
  • Coordinate care for a designated caseload of older adults with severe mental health conditions.
  • Work within the Care Programme Approach (CPA) policy and associated countywide risk procedures.
  • Travel independently to visit patients in their homes and community settings.
  • Use IT skills to maintain accurate records and communicate with colleagues.
  • Have a working knowledge of legislation regarding safeguarding vulnerable adults, the Mental Health Act, and the Mental Capacity Act.

Person Specification

We are looking for a nurse with experience in mental health, particularly in community settings. You will need to have a good understanding of severe mental illness and be able to work independently. Experience in later life interventions is desirable but not essential.
