About the Role

We are seeking a highly motivated and caring Mental Health Nurse to join our multi-disciplinary team at Gloucestershire Health and Care NHS Foundation Trust. As a Community Mental Health Nurse, you will play a vital role in providing high-quality care to older adults with severe mental health conditions.

Key Responsibilities
  • Coordinate a designated caseload of patients with complex mental health needs
  • Work within the Care Programme Approach (CPA) and associated countywide risk procedures
  • Travel independently to visit patients in their homes and community settings
  • Use IT skills to maintain accurate records and communicate with colleagues
  • Work in collaboration with other healthcare professionals to provide comprehensive care
Requirements
  • Experience in mental health nursing, preferably with older adults
  • Knowledge of legislation regarding safeguarding vulnerable adults, the Mental Health Act, and the Mental Capacity Act
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
